PSRK, PLLC, doing business under its registered trade name Restoration House ("Restoration House," "we," "us"), respects your privacy. This policy explains how we handle personal information collected through restorationhouse.health (the "site") and related communications before you become a patient.
Our Consumer Health Data Privacy Policy also applies and explains your rights under Washington's My Health My Data Act. Once you become a patient, your health care information is protected under Washington's Uniform Health Care Information Act (chapter 70.02 RCW) and, where it applies, HIPAA. It is governed by the notice of information practices we give patients, not by this policy.
Information We Collect
- Information you choose to send us: your name, email address, phone number, the area of care you are interested in, how you heard about us, and any note you include in our consult request form, an email, a phone call or a text.
- Technical information: our website host automatically processes information such as your IP address, browser type and the pages you visit, to deliver and secure the site.
How We Use It
- To respond to your inquiry and, if you ask, schedule a complimentary consult.
- To operate, maintain and secure the site.
- To comply with legal obligations.
We do not sell personal information, and we do not use it for targeted advertising.
Who We Share It With
We share personal information only with service providers that help us run the practice and may use it only on our behalf: website hosting and form processing, business email, and business phone and text services. Once you schedule as a patient, your information is managed in our practice management and health record system. We may also disclose information when the law requires it.
Calls and Text Messages
If you give us your mobile number, we may call or text you about your request. Message and data rates may apply. Reply STOP to opt out of texts at any time. We do not share mobile numbers or text-messaging consent with third parties for marketing.
Cookies and Tracking
We do not use advertising or social media tracking pixels, and we do not use third-party analytics that follow you across websites. Our website host may use strictly necessary cookies or similar technologies to deliver and secure the site.
How Long We Keep It
We keep inquiry information only as long as we need it to respond to you and for our ordinary business records, unless the law requires a longer period. You can ask us to delete it at any time.
Your Choices
You can ask us to access, correct or delete the personal information we hold about you by emailing info@restorationhouse.health. You may also have additional rights under Washington's My Health My Data Act, described in our Consumer Health Data Privacy Policy.
Security
We use reasonable administrative, technical and physical safeguards. No method of sending information over the internet is completely secure, so please don't include medical details in our contact form or in email.
Adults Only
Our site and services are intended for adults 18 and older. We do not knowingly collect personal information from children through the site.
Changes
We may update this policy. When we do, we will post the new version here with a new date.
