Consumer Health Data Privacy Policy
PSRK, PLLC, doing business under its registered trade name Restoration House ("Restoration House," "we," "us"), respects the privacy of your health information. This policy explains how we collect, use and share consumer health data, as defined by Washington's My Health My Data Act (RCW 19.373), through our website (restorationhouse.health) and related communications before you become a patient.
Once you become a patient, your health care information is protected under Washington's Uniform Health Care Information Act (chapter 70.02 RCW) and, where it applies, HIPAA. It is governed by the notice of information practices we give patients, not by this policy.
Consumer Health Data We Collect
- Contact information you provide: your name, email address and phone number, and, if you choose to tell us, how you heard about us.
- The area of care you are interested in, and any note you choose to include in our consult request form, an email, a phone call or a text. This can show that you are seeking health care services, which Washington law treats as consumer health data.
- Technical information, such as your IP address, browser type and the pages you visit, processed by our website host to deliver and secure the site.
Why We Collect It
- To respond to your inquiry and, if you ask, schedule a complimentary consult.
- To operate, maintain and secure the website.
- To comply with legal obligations.

We collect consumer health data directly from you (through our consult request form, email, phone or text) and automatically from your device when you use the website.
Consumer Health Data We Share, and With Whom
We do not sell consumer health data. We do not share it with any third parties or affiliates, so there are no categories of third parties or affiliates to list. We disclose the consumer health data described above (contact information, area of interest, notes and technical information) only to processors that handle it on our behalf under contract:
- Website hosting and form processing.
- Business email.
- Business phone and text messaging.
- Our practice management and electronic health record system, only once you schedule as a patient.
We do not share consumer health data with affiliates, advertisers or data brokers. We do not use advertising or social media tracking pixels, and we do not use geofencing around any health care location.
Your Rights
Under Washington law, you have the right to:
- Confirm whether we collect, share or sell your consumer health data, and access that data, including a list of all third parties and affiliates with which we have shared it and their contact information.
- Withdraw your consent to our collection and sharing of your consumer health data.
- Have your consumer health data deleted. We will also notify our processors to delete it.
To exercise these rights, email info@restorationhouse.health with the subject "Consumer Health Data Request," or call (564) 565-6524. We may need to verify your identity. We will respond within 45 days. We may extend that once by up to 45 more days when reasonably necessary, and we will tell you if we do. We will not discriminate against you for exercising your rights.
If we decline your request, you may appeal by replying to our decision with "Appeal" in the subject line. We will respond to your appeal within 45 days and explain our decision. If your appeal is denied, you may contact the Washington State Attorney General at www.atg.wa.gov/file-complaint.
Consent
We collect consumer health data through our consult request form with your consent, which you give by checking the box on the form. When you email, call or text us, or when our website host processes technical information to deliver the site, we collect only what is necessary to provide the service you asked for. You can withdraw consent at any time using the contact information below.
Changes
If we change this policy, we will post the new version here with a new date. We will ask for your consent before making material changes to how we collect or share consumer health data.
